Since you left from the Dominican Republic to the United States, and through unfortunate circumstances had to divert to FLL (I saw the aircraft parked at H8). Since FLL is the 1st port of arrival (through no means on anyone, but to assist a passenger in need of medical attention ASAP), yes you are required to clear customs and immigration in FLL before re-boarding to go to CLE.

If the final destination is in the U.S. and the first port of arrival is in the U.S., travelers must clear immigration and customs at the first port of entry.
